Bring a photo ID, your current medication list with dosages, and documentation from any specialists treating you for conditions like sleep apnea, diabetes, hypertension, or heart disease. If you wear glasses or hearing aids, bring those too.

Most DOT physicals are completed in 30 to 45 minutes. We respect your time and aim to get you back on the road quickly — with your medical certificate in hand whenever possible.

We accept walk-in DOT physicals when our schedule allows, but we recommend calling ahead at 765-792-7882 to confirm availability and avoid any wait time.
